• Age & Birthday of child(ren) (birthdays not needed for adult students)

• Class(es) you wish to register in

• *Chosen payment method (read policy letter for payment options)​

• Any accomodations that may be required

• Any other information that you feel is important (e.g. allergies, medical information, pronouns, etc.)
